History

The 5-2-1-0 messaging campaign was adapted from Let's Go! for use in Western Chula Vista via the Healthy Weight Collaborative. The Healthy Weight Collaborative is a project of the Health Resources Services Administration (HRSA) and the National Initiative for Children's Healthcare Quality (NICHQ). Our shared vision is to create partnerships between primary care, public health, and community organizations to discover sustainable ways to promote healthy weight and eliminate health disparities. In San Diego County, Healthy Weight Collaborative efforts are focused in the community of Western Chula Vista. Partners include:

  • Scripps Mercy Family Medicine Residency
  • Chula Vista Elementary School District
  • American Academy of Pediatrics, CA Chapter 3
  • County of San Diego Health & Human Services Agency
  • San Diego County Childhood Obesity Initiative, facilitated by Community Health Improvement Partners
  • Chula Vista Community Collaborative
  • YMCA Childcare Resource Service
  • Network for a Healthy California
  • University of California San Diego
